Physical Education Medical Form

All students registered in the schools of New York State are REQUIRED   by New York State Education Law and the Commissioner’s Regulations to attend courses of instruction in Physical Education (No Exemptions). These courses must be adapted to meet individual student needs if the student has medical limitations. This means that a student who is unable to participate fully in their Physical Education program must have activities modified to meet his/her individual needs. 

All long-term situations, which result in a doctor excusing your child from Physical Education, must include an alternate activity for your child. Please find a Physical Education Recommendation Form , which is located at the bottom of this page. This form should be completed by your child’s physician when a long-term excuse is given. Please return the form to the health office with your physician’s suggestions for limitations and the recommendations for participation in Physical Education. 

Your child’s Physical Education teacher will provide an educational plan for your child based on the physician’s recommendations. A parent may still excuse their child for a short term of 2 days, but we request any additional time be excused by your physician.

Faced once again with a rapidly spreading epidemic of mpox, the World Health Organization on Wednesday declared a global health emergency. The last time the W.H.O. made that call was in 2022, when the disease was still called monkeypox.

Ultimately the outbreak affected nearly 100,000 people worldwide, primarily gay and bisexual men, including more than 32,000 in the United States.

The W.H.O.’s decision this time was prompted by an escalating crisis of mpox concentrated in the Democratic Republic of Congo. It recently spread to a dozen other African countries. If it is not contained, the virus again may rampage all over the world, experts warned.

On Thursday, Sweden reported the first case of a deadlier form of mpox outside Africa , in a person who had traveled to the continent. “Occasional imported cases like the current one may continue to occur,” the country’s public health agency warned.

“There’s a need for concerted effort by all stakeholders, not only in Africa, but everywhere else,” Dr. Dimie Ogoina, a Nigerian scientist and chair of the W.H.O.’s mpox emergency committee, said on Wednesday.

Congo alone has reported 15,600 mpox cases and 537 deaths, most of them among children under 15, indicating that the nature of the disease and its mode of spread may have changed.

After private equity takes over hospitals, they are less able to care for patients, top medical researchers say

After private-equity firms acquire hospitals , the facilities’ assets and resources diminish significantly, leaving the facilities less equipped to care for patients, according to a new study by physician researchers at the University of California at San Francisco, Harvard Medical School and the City University of New York’s Hunter College.

Published Tuesday in the  Journal of the American Medical Association,  the research highlights a pattern of asset stripping at health care facilities purchased by private-equity firms, its researchers said, and is the first study to analyze the activity nationwide. 

“It’s a very striking finding and should change the way people think about private equity in hospitals,” said Dr. Stephanie  Woolhandler , a distinguished professor of public health at Hunter, part of CUNY, and one of seven authors of the study. “The PE firms say, ‘We bring new capital into hospitals.’ It turns out that’s not quite true.”

Assets studied in the research consist of land, buildings, major hospital equipment and information technology. The study found that during the two years after a private-equity acquisition, total capital assets at private equity-acquired hospitals declined by 15% on average while assets rose by an average of 9% at other hospitals. That makes for a net difference of 24%, the researchers found, equivalent to a loss of $28 million in total assets per hospital.The research studied 156 hospitals acquired by private-equity firms from 2010 to 2019 comparing them with 1,560 hospitals of similar size in similar communities that were not bought by private equity. The pattern of diminished assets persisted and widened five years after acquisition, the study reported.

Depleted assets translate to a reduced level of care, Woolhandler told NBC News, noting that equipment, buildings and technology are resources needed for patient care. “There are real dangers to the health care that people get if you deplete all the capital from a hospital,” she added.

Private-equity firms borrow money to buy companies that they hope to sell in a few years at a profit. Independent academic research shows that such leveraged buyouts result in far more bankruptcies than acquisitions that do not use so much debt and the deals can result in significant job losses for rank-and-file workers.Health care has been a primary focus of private-equity purchases in recent years, with over $500 billion invested in the industry by firms such as Apollo Global Management, The Blackstone Group, The Carlyle Group and KKR. The  American Investment Council , the industry lobbying group, says private equity improves health care. 

But increased patient falls and infections follow private-equity takeovers of hospitals, recent research shows, and residents of nursing homes owned by private-equity firms experience 10% greater mortality rates than those owned by other types of entities.

“Previous studies have found that patients are endangered and costs increase in the wake of private-equity acquisitions,” said Dr. Elizabeth Schrier, a resident physician at the University of California, San Francisco, and a lead author of the new research.

The report on asset-stripping in private equity-owned hospitals comes amid the collapse of Steward Health Care, a hospital chain recently owned by private equity that filed for bankruptcy in May, leaving patients and workers at 31 facilities adrift. Last  week , the Senate’s Health, Education, Labor and Pensions committee announced an investigation into the Steward crisis. 

Until 2020, Steward was owned by Cerberus Capital, a private-equity firm led by Steve Feinberg. In 2010, Cerberus bought a nonprofit chain known as Caritas Christi Health Care for around $250 million. The firm and its investor partners reaped an $800 million profit when it divested a decade later.

Along the way, Steward sold the land under its hospitals, generating a gain for investors but increasing the company’s costs significantly.

A Cerberus spokesman said in a  statement that it’s unfair and incorrect to characterize the Steward land sale as “looting” the company, as Sens. Ed Markey and Elizabeth Warren, both Massachusetts Democrats, have done. “During our nearly 11-year ownership of Steward, we supported the revitalization of failing community hospitals into a leading healthcare system,” the statement continued. “Cerberus’ long-term investment made it possible for Steward to continue to serve its communities, employ tens of thousands of professionals, and positively impact millions of patients’ lives.”

Amid rising numbers of health care takeovers by private equity, at least 10 states are ramping up  scrutiny  on the transactions to prevent patient harm, such as rising health care costs or the effects of monopolization. On July 1, Indiana began requiring that private-equity partnerships proposing transactions with health care companies valued at $10 million or more to notify the state’s attorney general 90 days before the proposed deal. While approval of the transaction is not mandatory, the attorney general can analyze antitrust concerns or issue a civil investigative demand for more information.

California, Connecticut, Illinois and Nevada are among the other states enacting new laws bringing scrutiny to private equity’s health care acquisitions.

Health care is not the only industry in which some private-equity owners have stripped companies’ assets. The Red Lobster restaurant chain  failed  in part because its private-equity owner sold off the company’s prime real estate, generating gains for itself. Meanwhile, Red Lobster had to pay rent on the properties, raising its costs and ultimately hobbling its operations.

Egypt Exempts Mandatory Assignment for Medical School Graduates

Starting 2025, Egyptian medical school graduates will be exempt from the mandatory two-year service program, known as ‘Takleef ,’ at primary healthcare facilities, according to Osama Abdel Hay, head of the Egyptian Medical Syndicate.

For over 50 years, Egyptian law has granted the Ministry of Health the right to mandate the assignment of medical school graduates to work in government hospitals or affiliated units. Thousands of Egyptians medical school graduates have been assigned since then. 

The new policy, introduced in September 2022, states that effective from 2025, placements will be determined based on need and staff shortages in hospitals and healthcare facilities. The policy applies to graduates in physical therapy, medicine, dentistry, nursing, and pharmacy, according to the General Syndicate of Physical Therapy.

The spokesperson of the Ministry of Health and Population, Hossam Abdel-Ghaffar, clarified that the decision to streamline, not eliminate, the mandatory assignment system for doctors was motivated by a range of factors. 

Among the main reasons is enhancing job satisfaction within the profession and financial considerations, according to Abdel-Ghaffar; the government allocates roughly EGP 500 million (USD 10.2 million) each year to sustain the assignment system, which includes expenses for salaries, logistics, and added incentives for doctors posted to less desirable areas around the country.

“This decision was necessary to prevent issues. It wouldn’t make sense for the ministry to require 1,000 pharmacists but assign 19,000. Many pharmacists might later request transfers to cities like Cairo, leaving health units and hospitals in places like Sohag understaffed,” a source at the Ministry of Health explained .

Khaled Amin, Assistant Secretary-General of the Egyptian Medical Syndicate, paralleled the same opinion, stating that a revision of the mandatory service system for medical graduates was necessary to better match medical education with job market needs.

The previous assignment system was the first step in a doctor’s career in Egypt within the health ministry. Doctors would serve in these assignments for one to three years, depending on their specialty, before they were eligible to explore other career opportunities or specializations.

The system aimed to ensure equitable healthcare access by assigning newly-graduated doctors to areas with the greatest need, often in remote or underserved regions such as Suhag and Minia. These doctors served as the primary source of medical care for these communities.

According to Abdel-Ghaffar, the mandatory system frequently caused dissatisfaction among young doctors, who felt that being assigned to locations regardless of their preferences or career goals resulted in lower-quality patient care. 

He said , “The new voluntary system gives doctors the right to choose positions that align with their skills and aspirations, ultimately improving job satisfaction and healthcare outcomes.” 

Shortage Sparks Need

The World Health Organization (WHO) estimates a minimum of 4.45 doctors, nurses, and midwives per 1,000 people is considered necessary to meet the healthcare workforce needs, as indicated by the Sustainable Development Goals (SDG) index threshold.

As of September 2020, the Egyptian Medical Syndicate reported that the Ministry of Health had 229,033 registered physicians, with only about 108,000 of these doctors working in the public health sector, excluding nurses, dentists, pharmacists, physical therapists, and lab technicians, according to a 2021 policy paper by the American University in Cairo (AUC).

This makes the ratio of physicians in Egypt 1.3 to 1,000 people, nearly half the recommended WHO average of 2.3 physicians to 1000 population, highlighting a stark shortage and need for physicians in Egypt. 

Many doctors leave Egypt to work abroad; as of Jan 2023, 4,361 government-employed doctors resigned, according to Ahmed Ali, a member of the Medical Syndicate Council. The Egyptian Medical Syndicate has blamed the phenomenon on low wages and a poor work environment, which includes understaffed and under-resourced medical facilities.

Egypt’s Minister of Health and Population, Khaled Abdel Ghaffar, echoed the same concerns, “There is a shortage of medical doctors relative to the population, although 14,000 doctors graduate from universities and are assigned to hospitals.”

The shortage, combined with assignments based on job market needs, suggests that while it is not mandatory for doctors to be assigned, they will still have the opportunity to complete their two-year service.

Medically Excused Program

Procedures and Program Information

Background Information: Students who are medically excused and cannot have their physical activities modified in the regular Physical Education program will be given written assignments to complete in an alternative setting. Students should report to their teacher and inform them of their medical circumstance.  The teacher will then send the student to the nurse to document the medical note.  Moving forward, sudents will report to the Media Center and sign in for daily attendance according to the length of their medical excuse. While in the Media Center, students will complete weekly assignments to demonstrate their understanding of physical education skills and content knowledge. Successful completion of the assignments will result in a letter grade and advancement to the next grade level per high school grading policy and graduation requirements. In some circumstances, the teacher may be able to involve the student in class activities while adhering to the medical restrictions.

Daily Procedures: After students have completed the daily attendance check in, they will log onto their school-issued device and visit the LHS H/PE department website to access their assignments. All assignments are for a one-week duration, as described on the assignment document. Students are to use their physical education period to complete their medical assignments. Students will abide by the rules and procedures of the Media Center during the term of their medical excuse. Be advised that the LHS attendance policy is in effect whether you are in the regular or medically excused physical education program. All rules and expectations will be enforced and infractions will be referred to LHS administration if not adhered to. If students have any questions related to their assignments, a grade or the procedures, they must contact his/her physical education teacher.

Additional Requirements and Information: By the end of day each Friday, students are expected to complete and turn in their assignments. All assignments must be emailed to the student's assigned physical education teacher with his/her name, assigned PE period, and the assignment's week number in the subject area. Unless there is a compelling reason or project option that has been discussed with the physical education teacher, all work is to be typed and emailed. If students have an assignment that requires a drop off or has a tangible product that they wish to present to the teacher, they may hand that piece to the Media Specialist or leave it in the teacher's mailbox. It is the student’s responsibility to confirm with the teacher that he/she received the assignment to avoid any lowering of grade or additional assignments. Failure to complete the assignments may result in a failing grade, lack of credit and having to repeat the grade level requirements for physical education.

Grading: Assignments will be graded by the teacher and given a letter grade as if the student were in the regular physical education program. Teachers will use a rubric to evaluate the work based on the requirements noted in the assignment description. Students should direct any questions to the physical education teacher.
