Jack ; John, Vanchit ; Batra, Chadni ; Batra, Chandni ; John, Vanchit ; Batra, Chandni ; Windsor, Jack Show more Background: Platelet derived growth factor (PDGF) has been shown since the late 1980s to play a major role in periodontal regeneration. PDGF has five isoforms and the isoform PDGF-BB has been found to be the most effective. Animal studies evaluating release kinetics of purified recombinant human platelet-derived growth factor (rh-PDGF-BB), demonstrated that 90% of rh-PDGF-BB was depleted from sites within 72 hours after implantation. Thus, the aim of this in-vitro study was to evaluate the effects of two rhPDGF-BB applications on periodontal ligament fibroblasts proliferation and VEGF expression. Materials & Methods: Periodontal ligament (PDL) cells were seeded in multiple 6 well plates and split into 3 groups. Group 1 served as a control that received no rh-PDGF-BB. Group 2 was given an initial dose of 10 ng/ml rhPDGF-BB on day 1 and none on day 3. Group 3 received a dose of 10 ng/ml rhPDGF-BB on day 1 and a second dose on day 3. On day 3, half of the plates were stopped and media collected was used for LDH, WST-1 and VEGF ELISA analysis for evaluation of VEGF expression, cellular proliferation and cytotoxicity. Plain media was then added to groups 1 and 2 while group 3 received media with rh-PDGF-BB. On day 6, the collected media was used for ELISA, WST-1 and LDH assays. Results: PDL cell proliferation was not significantly different between any group studied (p=0.689). VEGF expression was increased in group 2 and 3 on day 3 compared to group 1 (p=0.005). There was no difference between groups 1 and 2 on day 6 (p=0.977). Conclusion: The addition of rhPDGF-BB to PDL cells did not increase proliferation at any time point. A single application of rhPDGF-BB increased the expression of VEGF from PDL cells; however, an additional application did not significantly increase the expression of VEGF. Show more

Background: The literature indicates a reduced survival rate for dental implants placed at previously failed sites, smokers, and patients with a history of periodontal disease. The aim of this study is to review the available literature reporting on the success and/or survival of rough surface implants placed at previously failed sites, in smokers, and in periodontally compromised patients. An attempt was made to systematically review the literature and calculate an overall weighted mean survival rate for rough surface implants in each of the above three scenarios.

Methods: An electronic literature search (MEDLINE-PubMed) was performed and references hand-searched for human studies addressing the success/survival of implants placed at previously failed sites, in smokers and in patients with a history of chronic periodontal disease. The overall weighted mean survival rates and 95% confidence interval were then calculated. Results: Six retrospective studies reporting on implants placed at previously failed sites were included with total of 343 second attempts and 31 third attempts at implant placement at failed sites in 330 patients. The weighted mean survival rates for the second and third attempts at implant placement in a previously failed site were calculated to be 88.05% and 74.19%, respectively. A total of 14,395 implants were included in smoking analysis. Of these, 10,403 implants were placed in non-smokers with 250 failures and 3,992 placed in smokers, with 205 failures. The calculated overall weighted mean implant-level survival was 97.67% in non-smokers and 95.03% for in smokers. Lastly, six studies reported on 591 implants placed in periodontally compromised patients with a total of 15 failures and 198 implants placed in periodontally healthy patients with just one failure. The overall weighted mean implant survival rate was calculated to be 97.48% in periodontally compromised patients compared to 99.49% for periodontally healthy patients.

Conclusions: Of the three risk factors evaluated in this review, rough surface implants placed in previously failed sites presents the highest risk for implant failure. Rough surface implant survival declines significantly for each additional attempt at implant placement at a previously failed site, with weighted mean survival rates of 88.05% and 74.19% for the second and third attempts, respectively. Although more favorable than previously used implant designs, smokers continue to experience lower survival rates compared to non-smokers, with overall weighted mean implant survival rates of 95.03% and 97.67%, respectively. A similar finding was found for implants placed in patients with a history of chronic periodontitis. The calculated weighted mean survival rate was 99.49% for periodontally healthy patients compared to 97.48% for periodontally compromised patients.

A study was carried in conjunction with the Orthodontic department at Halmstad General Hospital in Sweden to investigate the possibility of reducing cost and manufacture time of dental implant drill guides.

The current system involves sending a digital image in STL format to the Materialise factory in Belgium where information of the position of dental implants is translated onto a moulded mouthpiece. Drill guides are placed in the mouth piece which is then returned to the surgeon. The mouthpiece complete with drill guides is then placed in the patients mouth and used as a guide for the implant drill holes. The cost of 10000 sek and a turnaround time of 2 weeks gave rise to the need for a faster and cheaper solution.

A new mouthpiece was designed comprising of a solid cube which could be clearly seen on the x-ray. Linearisation of the cube faces is used to find a reference point from which to drive a 5 axis drilling platform. The mouthpiece is placed in the drill platform which is driven by stepper motors which in turn are controlled by a microcontroller. Co-ordinates are entered via a PC interface. The PC software then translates these co-ordinates into motor steps which are sent to the microcontroller. The drill platform then positions the mouthpiece in order to drill guide holes for the dental implants.

The study showed that the machine design gave an acceptable degree of accuracy and repeatability. Further enhancements could be made by automating the detection of the cube using image analysis techniques. The study was also limited by the lack of graphical and geometrical data concerning the position of the implant. For the purpose of this study the co-ordinates for the implants guides is entered by hand.

It was concluded that further software and hardware enhancement would be needed before the application could be developed commercially.

Q&A: Researcher discusses autism in young adulthood

Our understanding of autism has changed significantly in the past years. Previously viewed as a rare childhood disorder with a focus on deficits and pathology, autism is now recognized as a common, lifelong and heterogenous condition characterized by a broad spectrum of strengths and challenges. With increasing numbers of autistic adults and in particular females, a better understanding of autism in young adulthood for autistic females and males is warranted.

In her thesis, Miriam Martini, Ph.D. student at the Department of Medical Epidemiology and Biostatistics, Karolinska Institutet, aimed to gain a better understanding of the factors that contribute to autism continuing into adulthood. She also aimed to identify which mental and physical health problems autistic females and males might be struggling with around the timing of autism diagnosis and during the challenging transition to adulthood, and how mental and physical health problems influence each other. By comparing autistic individuals to nonautistic individuals and exploring how autistic females and males differ from each other, the thesis aimed to better support the needs of the autistic community.

In this interview, she discusses her findings.

What are the most important results in your thesis?

We see that genetic factors might be important across development. Autism is highly heritable from childhood into adulthood, with genetic factors contributing to its stability over time. Interestingly, while some genetic influences remain stable over time, new ones can emerge as individuals age. Importantly, the thesis highlights complex health challenges in autistic individuals, particularly with regards to mental health problems, as they transition to adulthood.

As a group, autistic individuals are at a higher risk to be diagnosed with or hospitalized for psychiatric difficulties compared to nonautistic individuals between ages 16 and 25. We also show associations between childhood somatic conditions and psychiatric conditions between 16 and 25, as well as associations between childhood psychiatric conditions and adult somatic conditions in autistic individuals. Regarding sex differences, we found autistic females to be particularly at risk. Compared to autistic males, they show higher rates of psychiatric diagnoses before autism diagnosis that seem more persistent and show an increased risk for psychiatric diagnoses and hospitalizations in young adulthood.

Why did you become interested in this topic?

My interest in this topic stems from my background in psychology and an internship in autism research during my studies, where I became particularly interested in autism beyond childhood and its strong association with mental health. As I learned more about the gender health data gap, I recognized the unique opportunity large register data in Sweden provides for epidemiological research, especially in examining sex differences.

I am very passionate about improving mental health outcomes, particularly for those who face significant challenges in this area, and I hope my work contributes to better mental health care and support for autistic individuals, and particularly autistic females, who are often overlooked or excluded from services because of various reasons.

What do you think should be done in future research?

There is a critical need to expand mental health services for autistic individuals, particularly during the transition to adulthood . Additionally, care must be tailored to address the specific needs of both autistic females and males, ultimately improving their long-term health and quality of life.

To achieve this, health care providers require more education on autism and neurodevelopmental conditions to better recognize the unique manifestations of mental and physical health issues in autistic individuals, which will improve differential diagnosis and treatment of these co-occurring conditions.

Collaboration with the autistic community, caregivers, and health care professionals is essential to ensure research translates into practical, person-centered care strategies, meeting the complex needs of autistic individuals across their lifespans.
