The long‐term goal for integrated information management at the Johns Hopkins Medical Institutions is the creation of a knowledge management environment—a network of databases that an individual would tap as one would one's own memory. An important library component in this emerging environment is WELMED, a general purpose bibliographic database management system that permits the library to distribute its databases and related services to its users through common, customizable interfaces. A library service wants to create a database to store details of its libraries, books, and borrowers. Details include the following: A book has a unique ISBN number, a title and one or more authors. The library service may own several copies of a given book, each of which is in one of the service’s libraries. A given library contains many books, and to distinguish different copies of the same book a library assigns a different copy-number to each of its copies of a given book; the price that was paid for each copy is also recorded. Every library has a unique name and is either a main library or a branch library. A main library may have zero or more branch libraries and every branch library is a branch of exactly one main library. A borrower has a name and a unique ID code. A borrower can have many books on loan, but each copy of a book can only be on loan to one borrower. A borrower could borrow the same book on several occasions, but it is assumed that each such loan will take place on a different date. The main operation and the details are included here & the developers can suggest some new options and services for the above system. To implement the new and advanced options you can assume any database related structures and constrains.

This study was carried out to design and introduce the use of an automated clinic record management system and has chosen Ahmadu Bello University Sick-bay as the case study for the first implementation with the aim of improving their services especially in this digital era. A qualitative research was adopted and the instrument used for the study are interview, observation and questionnaire where some questions were raised by the researcher such as, what are the challenges associated with the use of the current manual record management system in the Ahmadu Bello University Sick-bay? How to improve on the current manual record managements system to minimize human error in the process of the records? How to improve on the current manual time management among the staff of the clinic? And how to provide more privacy to patients’ records and information in the clinic? Fifteen (15) respondents were randomly chosen from both the clients and staff of the clinic. And at the end of the research, the findings were analyzed which led to the designing of the aforementioned system. And in order to design the programme, the researcher uses some web development tools such as; Adobe Dreamweaver and Notepad Plus Plus for the required coding; hypertext preprocessor (PHP) and Java scripts (JS) to connect the interfaces to the database; and My-SQL was used to design the databases that store the overall system data. Finally; summary, conclusion and suggestion were given on the problems associated with the manual method of record keeping such as difficulties in sorting, retrieving and updating records, lack of security of records, loss of relevant information and so on, it was also suggested that the implementation of this programme will help in improving the effectiveness and efficiency in our clinics.

The need for effective health care in Nigeria as in many other countries is face with growing demand for effective health care services due to growing population and higher standard for the quality of life, the study was conducted through cross-sectional design. It aim at assessing the best practices in patient data management for effective health care service delivery.Data was collected from Doctors, Nurses, medical laboratory staff, medical record staff and patients. Patients were interviewed in the selected tertiary hospitals of Jigawa state. A total of five hundred (500) medical personnel and patients were involved in the selected tertiary hospitals in Jigawa state of Nigeria. Data were analyzed using descriptive statistics. The Findings revealed manual method is the method used at the selected tertiary hospitals in capturing patient information. This finding is in agreement with study from Nigeria that showed that there was no awareness of the use of computers for record management in hospitals and this affected EMR technology adoption. It was concluded that the use of electronic health record will increase the efficiency of health if implemented according to the best practices.

Abstract-We live in an era when manual systems are becoming obsolete and being replaced by e-platforms for enhanced efficiency and better productivity. In this paper, an attempt was made to develop an automated record management system for a typical health institution. The record systems of five hospitals in south-west Nigeria were used as case studies for evolving a robust and comprehensive design for the electronic database. Java programming language (NetBeans IDE 6.7.1) was used to develop the application while MySQL was used as the query language. The application offers numerous advantages over the traditional manual record-keeping system among which are data consistency, data integrity, ease of data retrieval, record integration and data mining.

Patient record management systems in hospital today necessitate a competent administration when handling patients, generating reports from cashier, patient details which serves as a key factor for the flow of business transactions in Kinoni Health center. Unfortunately, the current Record management system leads to misplacement of drug details, payment details, and late release of reports and insecurity to records. This research project is aimed at computerizing all the records about patients, staff and drug suppliers. In order to achieve this goal, a thorough System Study and investigation was carried out and data was collected and analyzed about the current system using document and data flow diagrams.
