How do I write a case study?

The easiest way to write a case study is to get started with a template. This provides you with a pre-set cover page and table of contents; a variety of page layouts to work with; and a picture of how to organize content, add designs and break up text. A case study is not a technical document that needs to be structured in a specific and formal way. You can get creative but focus on making your content clear and easy to understand.

What's an example of a case study?

Case studies, in business and marketing, are stories of success achieved through a product or service. The product can even be a strategy or framework that was pioneered by an industry thought leader. Many brands publish case studies on their website to share how their clients see tremendous value in using their products. The case study essentially chronicles the entire client journey from having a problem, to finding the solution, and the outcome of that solution.

What is a Case Study

A case study is like a detailed story that looks closely at a particular situation, person, or event, especially in the business world. It's a way to understand how things work in real life and learn valuable lessons. For instance, if a business wanted to figure out how another one became successful, they might study that business as a case study.

Let's say there's a small company that started selling handmade products online and became successful. A case study about this business could explain the challenges they faced, the strategies they used to grow, and the results they achieved. By reading this case study, other businesses could learn useful tips and apply them to their situations to improve and succeed.

7 Tips For Writing Great Case Studies

  • Pick a Familiar Topic: Choose a client or project that your audience can relate to. This makes it easier for them to see how your solutions might work for their situations.
  • Clear Structure: Start with a concise introduction that sets the stage for the case study. Clearly outline the problem, solution, and results to make your case study easy to follow.
  • Engaging Storytelling: Turn your case study into a compelling narrative. Use real-world examples, anecdotes, and quotes to make it relatable and interesting for your audience.
  • Focus on the Problem: Clearly define the problem or challenge your case study addresses. This helps readers understand the context and sets the foundation for the solution.
  • Highlight Solutions: Showcase the strategies or solutions implemented to overcome the problem. Provide details about the process, tools used, and any unique approaches that contributed to the success.
  • Optimize for SEO: By incorporating your case study into a blog post using a blog post generator, you enhance its visibility and reach. This, in turn, improves the search engine rankings of your blog post, attracting more organic traffic.
  • Quantify Results: Use data and metrics to quantify the impact of your solutions. Whether it's increased revenue, improved efficiency, or customer satisfaction, concrete results add credibility and demonstrate the value of your case study.

When it comes to high impact marketing content, case studies are at the top of the list for helping show off your brand’s stuff.

In this post, I’ve put together a few high-level case study design tips as well as 14 professionally designed case study templates that you can use to start designing beautiful case studies today.

Let’s begin!

Here’s a short selection of 12 easy-to-edit case study templates you can edit, share and download with Visme. View more templates below:

What is a Case Study?

A case study is a way for you to demonstrate the success you’ve already had with existing clients. When you create a case study, you explore how previous clients have used your product or service to reach their goals.

In particular, a case study highlights a specific challenge or goal one of your clients was struggling with before they discovered your product.

It then demonstrates how your work has assisted them on the journey towards overcoming the challenge or accomplishing the goal.

A case study’s outcome is typically to share the story of a company’s growth or highlight the increase of metrics the company tracks to understand success.

The case study includes an analysis of a campaign or project that goes through a few steps from identifying the problem to how you implemented the solution.

How to Write a Case Study

When it comes to adding irresistible design to your content from the start, using a helpful tool is a great start. Sign up for a free Visme account and start highlighting your own client success stories using one of our case study templates today.

Also, while you’re beginning to transition your case study workflow to include a professional design tool, it’s helpful to review some high level principles you can incorporate into your case study.

We’ll start by reviewing some of the critical style tips and structural elements to include in your case study before progressing to a more detailed design section.

An infographic sharing three style tips for case studies.

Pinpoint Your Main Message

When designing an impactful case study, it’s essential to stay clear on the metrics that you’re highlighting. The process of overcoming business challenges is a dynamic process with many moving parts.

If you do not stay focused on what matters in your case study, you risk obscuring the big win your client experienced by using your product or service.

This is why you need to focus on a single message or metric. This is often called the north star metric .

The north star metric is the single most crucial rate, count or ratio that helped your client move closer towards their goals or overcame an obstacle.

While north star metrics are context dependent, a useful heuristic you can utilize is to figure out the most predictive metric of your client’s long term success.

In the template I’ll highlight below, cost per lead was the north star metric that The College for Adult Learning needed to optimize.

Use Emotionally Rich Language

Recently researchers at Presado did an interesting study to understand the types of language that help readers take action. They broke the content included in marketing assets into several categories, including functional, emotional and descriptive.

In the most successful pieces of content, the researchers found that emotionally coded words were present in 61% of the content’s total volume.

This research shows the benefit of using emotionally engaging content in your case study. While it’s essential to focus on the concrete evidence of how you helped your client get from where they started to a successful outcome, do not forget to highlight the emotional journey.

A diagram showcasing marketing language and the emotions it evokes.

Use Data For Concrete Evidence

Once you’ve decided on the north star metric to highlight and you choose the emotional response you want to reinforce in your case study, it’s important to use actual data from the project to share the concrete results your product helped to achieve.

To make sure your audience can follow your line of thinking, make sure the data in your case study is precise. If you track data across time, your readers must know whether you chose to track by month or years.

If there are any apparent trends, you can use color to highlight specific areas in a chart.

If you want to dig deeper into using data to tell compelling stories, check out our video data storytelling tips to improve your charts and graphs.

In the template below, The College for Adult Learning case study is an excellent example of how these elements can work together.

Cost per lead was a critical north star metric, so we chose to emphasize the increase in revenue and a decrease in cost per lead.

Additionally, the background section uses emotionally rich language by highlighting how the school helps students get ahead with their career goals. Also, the factual data is the centerpiece of this page in the case study.

If you’re ready to share how you impacted a client, use the College for Adult Learning case study template right now!

Include All Necessary Parts of a Case Study

After you’ve interviewed your client and you’re getting ready to start writing, it’s important to remember each piece you need to cover.

All good case studies consist of five parts: Introduction, Challenge, Solution, Benefit and Result.

An informational infographic template showcasing parts of a case study available to customize in Visme.

While you don’t necessarily need to label each section like that, be sure that the flow makes sense and covers each section fully to give your audience the full scope of your case study.

Case Study Design Tips

Now that we’ve explored the 14 templates you can use with Visme to create your case study, let’s take a look at some practical design tips that will take your content to the next level.

Infographic sharing six case study design tips.

Be Brief In Your Case Study

In discussions about writing with style, brevity is a common topic. However, it’s also an important design principle.

Brevity in design is when you find the best way to perform your intended objective in as few steps as possible.

When designing your case study, make sure you do not add extraneous visual elements where they are not needed. Instead, think of the effect you want to have on your reader and try to do it simply.

Describe Your Vision Clearly

Earlier in this article, I wrote about the north star metric, your case study’s emotional effect and using data to make the case study concrete. Your design choices should serve to reinforce these primary goals.

Clarity in design is when all of the visual elements add up to a whole.

A great example of this is in the small business case study template where the shapes, typography and color scheme all emphasize the main idea that Visme helps the reader save time.

Blue and purple presentation slide showcasing the highlight of a case study.

Create A Consistent Style

Visual consistency is a fundamental design principle that you can not afford to ignore in your case study. It will help you increase readability and make sure your audience does not get frustrated with jarring visual elements.

In short, a consistent style is when you use a uniform color scheme, typography and the same kinds of visual elements throughout the case study.

Use A Case Study Template For Readability

Readability is a crucial element of design, especially for case studies that are experienced on mobile devices. Contrast is an impactful readability principle.

Make sure any contrasting colors you chose are easy on the eye and your reader does not have to strain to read your case study.

Use Proper Alignment In Your Case Study

Alignment is one of the principles of design that sets professionally designed business case study templates apart. Great designers have an intuitive eye for the mathematically based ratios of proximity invisible in sound design and an eyesore in lousy design.

The good news is that you do not have to be a mathematician nor a professional designer to have a perfect alignment for your case study. Visme utilizes an easy to use drag and drop design tool that helps you achieve proper alignment in your case study.

Let Your Brand Personality Speak

When we make intentional design decisions, we want to create a positive emotional experience for our audience. One of the best ways to do that is to make decisions that showcase your brand’s unique personality .

Is the case study you are creating like a well dressed business person who is serious, trustworthy and capable of doing a great job? Is it more like an extravert at a party bouncing from person to person lighting up the room?

There is no right answer, but you need to infuse your viewpoint into the case study you create if you want to create a unique design.

Start Designing Your Case Study Today

A professionally designed case study template will help you create a stunning case study. While reviewing some high level design strategies is an important step, a tool like Visme will help you make a real impact on your audience.

15 excellent ux case studies every creative should read.

  • By Sandra Boicheva
  • October 21st, 2021

In a previous article, we talked about UX portfolios and how they carefully craft a story of how designers work. Interestingly enough, recruiters decide if a UX freelance designer or an agency is a good match within 5 minutes into the portfolio . In order to persuade these recruiters, the portfolio needs to present an appealing story that showcases the skill, the thought process, and the choices taken for key parts of the designs. With this in mind, today we’ll talk about UX case studies and give 15 excellent examples of case studies with compelling stories.

The Storytelling Approach in UX Case Studies

An essential part of the portfolio of a UX designer is the case studies that pack a showcase of the designer’s skills, way of thinking, insights in the form of compelling stories. These case studies are often the selling point as recruiters look for freelancers and agencies who can communicate their ideas through design and explain themselves in a clear and appealing way. So how does this work?

Photography by Alvaro Reyes

Just like with every other story, UX case studies also start with an introduction, have a middle, and end with a conclusion .

  • Introduction: This UX case study example starts with a design brief and presents the main challenges and requirements. In short, the UX designer presents the problem, their solution, and their role.
  • Middle: The actual story of the case study example explains the design process and the techniques used. This usually starts with obstacles, design thinking, research, and unexpected challenges. All these elements lead to the best part of the story: the action part. It is where the story unveils the designer’s insights, ideas, choices, testing, and decisions.
  • Conclusion: The final reveal shows the results and gives space for reflection where the designer explains what they’ve learned, and what they’ve achieved.

Now as we gave you the introduction, let’s get to the main storyline and enjoy 15 UX case studies that tell a compelling story.

1. Car Dealer Website for Mercedes-Benz Ukraine by Fulcrum

13. An Approach to Digitization in Education by Moritz Oesterlau

This case study is for an online platform for challenge-based learning. The designer’s role was to create an entire product design from research to conception, visualization, and testing. This is a very important case study and there’s a lot to take from it. First, the project was too ambitious and the goal was too big and vague. Although the result is rather an approximation and, above all, at the conceptual level requires further work, the case study is incredibly insightful, informative, and insightful.

  • Interactions: explaining the main interactions and the decisions behind them.
  • Shared Design Language: Explaining the decision to provide links on each product page so users could be directed to their preferred retailer to place their order. Including recommended products to provide users with alternatives.
  • Outcome and Learning: The good ending.
  • Project Information: Listing all stakeholders, the UX designer’s role in a bullet list, and design tools.

In Conclusion

These were the 15 UX case studies we wanted to share with you as they all tell their story differently. If we can take something valuable about what are the best practices for making an outstanding case study, it will be something like this.

Just like with literature, storytelling isn’t a blueprint: you can write short stories, long in-depth analyses, or create a visual novel to show your story rather than tell. The detailed in-depth UX case studies with lots of insights aren’t superior to the shorter visual ones or vice versa. What’s important is for a case study to give a comprehensive view of the process, challenges, decisions, and design thinking behind the completed project .

In conclusion, a UX case study should always include a summary; the challenges; the personas; roles and responsibilities; the process; as well as the outcomes, and lessons learned.

Expert tips, what is a case study.

A case study is an in-depth look at an individual or organization. Often they’re used to show the value of a product or service with real-life examples. Your company might walk potential customers through a breakdown of your current client’s success with a particular product. That can be a compelling way to market your products and grow your business.

Case studies are often found on company websites or white papers—or presented directly to potential clients. The key is creating an easily digestible document or presentation that succinctly conveys the value of your product or service. A case study template is a quick way to craft a compelling case study that’s ready to show to clients.

How long should a case study be?

Often a case study is a written document. This is the case when it’s embedded in a white paper or another promotional document. The traditional length for these case studies is 500–1,000 words. That length can provide enough detail without overwhelming the reader.

However, your case studies don’t need to be a full-length document. Instead, consider creating a visual presentation. This format can help you create a powerful narrative that’s going to stick with your audience. After all, images and numbers are more memorable. Utilizing them within your case study can help you capture the value you’re trying to show. Try this case study template to see how compelling your case study can become.

How to write a case study

Let’s dive into how to create your own case study using this Tome template.

Write a compelling headline.

  • Start with a headline that immediately tells your audience exactly what they can expect. Think of it like an email subject line. You want to convey your main message in only a sentence—that way your readers understand exactly what’s inside even if they read no further. This case study template draws attention to the headline and makes it front and center.

Give an overview of the case study.

  • Next up is the overview slide. This section is an opportunity to briefly summarize the main point of the entire case study. The overview is also your chance to introduce some key stats. Adding numbers is a powerful way to show the concrete value of your products. Potential buyers can see exactly what benefits the case study client experienced.
  • The Tome case study template provides you with spaces for a brief paragraph, some key numbers, and context for the stats. It’s designed to give your clients a snapshot of the entire presentation.

Introduce the client.

  • After setting the stage, it’s time to explain who the client in the case study is. Outline what they do and their target customers. You might even outline the demographics of their audience. Explain some key characteristics of their business strategy and how they were performing prior to working with your company. The Tome template provides an easy way to showcase your star customer.

Present the problem.

The client offered financial services to their users. They needed to ensure that the transactions within the app were fast and reliable. A lot of data needs to be stored. It is important to find a more secure and authentic way for users to interact with the app.

We used the latest technologies Blockchain and 5G to enable transparency and security. This technology also helped us establish security and transparency via encryption and smart contracts.

As the client also wanted to ensure fast transactions, we implemented 5G technology. This allowed high-speed and low-latency connectivity. As a result, the transactions were efficient, high-speed and convenient to the users of our client.

4 apps all students should have on their iPhones

a hand holds a phone as heart emojis appear to float out to the screen

There's more to consider than just what supplies to buy this year (though we'll help with that too.)

Navigating college life can feel like a juggling act with endless assignments, tight schedules, and a social life to maintain. So that's why we’ve curated four apps that every college student should have in their arsenal.

From note-taking to productivity to citation management, these apps are designed to streamline your studies, keep you on top of your tasks, and look pretty too.

1. Google Docs

When it comes time to get assignments done, the Google Workspace apps are essential. These services include Docs, Drive, Sheets, and Slides, which are simpler to use across mobile and desktop than Microsoft Office Suite (now known as Microsoft 365).

Google Docs is ideal for writing essays and collaborating in real-time, while Sheets makes managing data and creating spreadsheets a breeze. For presentations, Slides is your go-to for making polished demonstrations without the added learning curve of PowerPoint.

Google Drive ( which has its own app ) then ties everything together, providing seamless storage and sharing across all devices. You can even scan your assignments with Drive which is essential for those professors who require you to upload printed homework onto Canvas or Blackboard.

Unlike other note-taking or task-management apps, Notion combines it all. Notes, to-do lists, calendars, and databases, its drag-and-drop interface makes it easy to customize your workspace to fit your needs. While other apps might focus on one feature—like Evernote for note-taking or Trello for task management—Notion offers a complete package.

This all-in-one approach is perfect for students who want to stay organized and efficient without the hassle of switching between multiple apps, giving them more time to focus on their schoolwork. Although it does have a subscription tier, you can use the core of its features for free, and who doesn't love free?

3. Bear Notes

This beautifully designed app offers a distraction-free interface, perfect for jotting down lecture notes, brainstorming ideas, or crafting the perfect essay. Its clean, intuitive design makes it easy to stay focused and organized.

How a Mission to Cut Food Waste Launched a Multimillion-Dollar Venture

On a hectic Friday in October 2016, Josh Domingues wondered if he had made a mistake quitting the security of a well-paying job managing contracts for professional hockey players to start a new venture selling nearly expired groceries at discount prices. After all, a trial run of the 27-year-old entrepreneur’s Flashfood app at a Toronto supermarket hadn’t gone well, ending with confusion among customers and ire from the store manager.

But Domingues, who had launched the app after learning that international food waste was the third-largest contributor to greenhouse gas emissions, remained focused on his mission to reduce waste—and eight years later, his risk has paid off.

The Flashfood app is now being tapped by customers in more than 2,000 supermarkets in the United States and Canada. What’s more, the app has kept 65 million pounds of almost-out-of-date produce, meat, and other food items from being thrown into landfills, and 130 million pounds of greenhouse gas emissions from that food out of the atmosphere.

“Josh made the decision to commit to an idea that is not proprietary at all … in a competitive space with razor-thin margins, and yet he made it work.”

Domingues is a fresh example of the growing number of social entrepreneurs whose credo is “putting purpose before profit,” as Harvard Business School Senior Lecturer and serial entrepreneur Reza Satchu chronicles in a 2024 case study, “Flashfood: The Magic of Commitment.”

Satchu, a senior lecturer in the Entrepreneurial Management Unit, is founder and managing partner of the investment management company Alignvest Management Corporation and founding chairman of the NEXT Canada network of entrepreneurs and investors. He says many hopeful entrepreneurs can learn important lessons from Domingues’ persistence.

“What's remarkable is not that Josh had the idea,” says Satchu, who taught Domingues as a student in a NEXT Canada program years ago. “It’s that he trusted his judgement to solve a real problem, and he's now got customers all over North America. Josh made the decision to commit to an idea that is not proprietary at all … in a competitive space with razor-thin margins, and yet he made it work.”

‘People need this’

Like many other social entrepreneurs, Domingues’ road to success was not easy. He took a winding path from playing wing for a minor league hockey team in Canada to working in management consulting before trying, failing, and trying again to succeed with the Flashfood app.

Food waste wasn’t on Domingues’ mind until 2016, when his sister, a caterer, told him that she had to throw out $4,000 worth of leftover food from a corporate event. Troubled by the waste, he researched the issue and learned that the world’s wasted food could feed 800 million people. To test the idea of selling less-than-perfect products at a discounted price, he bought $200 worth of “unpretty” food, set up a sales table at a busy Toronto intersection, and sold all items for $1 apiece in 20 minutes. “People need this,” he realized.

“Yet somehow, he committed, and it was through that commitment that the magic happened.”

“A former executive at one of the big grocery chains in Canada told me that across 39 stores the year before, they threw out something like $110 million worth of food,” Domingues says in the case. “I remember just being struck and thinking, ‘What are you doing?’”

Soon afterward, Domingues began pitching his idea to grocery chains: Customers would use a mobile app to buy food at a discount before it got discarded. Customers could therefore acquire food at cheaper prices and grocery stores could gain profit from food they otherwise would have tossed. Domingues would charge stores a 5 percent service fee.

After a string of rejections, he persuaded the owner of Farm Boy, a high-end Canadian grocery chain, to let him sell aging food for half price from a kiosk inside a store. Domingues hired a tech service firm to design an app, and two weeks later, he launched Flashfood.

“He had no expertise around food. He didn't know how to build a mobile app. He had no resources or capital,” Satchu says. “Yet somehow, he committed, and it was through that commitment that the magic happened.”

Was it time to give up?

It took a while for the magic to materialize. The Farm Boy pilot broke down on the third day, as most customers went directly to the in-store kiosk without downloading the app. The resulting congestion of regular shoppers and kiosk customers earned Domingues a lecture from the store manager, and the partners agreed to pause the pilot.

In the ensuing months, Domingues investigated two other possible ways to sell leftover food: collect discards from restaurants, or work with one of Canada’s largest greenhouse farmers to develop direct, farm-to-customer sales. He soon saw that restaurants posed two problems: transportation costs and legal liability, since most of the food would be leftovers from diners’ plates. The farm-to-customer model was appealing, but would take time to expand. After much deliberation, Domingues decided to stick with the grocery app.

“He could scale much faster,” Satchu says of the app. “You can get 100 or 1,000 stores.” He says Domingues also knew it was crucial to be the first such venture to partner with a supermarket chain because “once you're in, it's hard to be replaced [by a competitor].”

As of December 2023, Flashfood was available in more than 2,000 supermarkets, including 700 Loblaws stores in Canada and 300 Stop & Shop supermarkets in New England, New York, and New Jersey. Ahold Delhaize’s Stop & Shop partnered with Flashfood in 2021, and the company says that since then, it has saved a million pounds of food from landfills and has saved shoppers more than $2 million on groceries. As of 2023, privately owned Flashfood had over 100 employees and approximately $15 million in revenue. Marking a new stage in the company’s evolution, Domingues stepped down as CEO earlier this year and now serves as executive chairman.

Lessons in social entrepreneurship

To Satchu, Domingues’ experience offers three significant takeaways for hopeful prospective founders seeking to make a social impact:

  • There’s no ideal path to success. “People perceive there is this optimal path to becoming a founder, and it's just not true,” he says. “Being a founder is all about maximizing learning and learning is all about making consequential decisions. Have you led a life where you've learned a lot?”
  • Act on your vision now, not later. “A lot of people go to business school and take a track where they state, ‘I'll make a lot of money, and then I'll focus on social enterprise.’ Right? But you may never actually get to the point where you do the things you want to do, so you need to live a life where hopefully, it's intertwined.”
  • A sense of mission and purpose are assets. “Magic happens when you commit because that is when people most want to help you. They think you want to make the world a better place, [so] they make a bet on you, and are your first employee, your first customer.”

Satchu stresses the importance for entrepreneurs to explore new ideas, even those that may initially seem far-fetched.

“Someone like Josh is always going to have lots of ideas. He could immediately dismiss those ideas … or he can spend 20 minutes to explore them. It may be a dumb idea, in which case he's wasted 20 minutes of his life. Or it may be the next Flashfood.”

Background: During the COVID-19 pandemic period, it was difficult to carry out regular and scheduled follow-up of patients in the outpatient department, especially during lockdown periods. However, early detection of initial infection or other serious conditions is vital for patients after ocular surgery, such as intravitreal injection (IVI) for age-related macular degeneration (AMD).

Objective: We evaluated the use of a smartphone-based postoperative care chatbot system (PCCS) with an instant bidirectional feedback system for patients to self-report postoperative symptoms and signs.

Methods: During the COVID-19 level 3 epidemic alert in July 2021 in Taiwan, the PCCS alerted the patients to report and grade 6 ocular symptoms and signs associated with ocular inflammation or retinal detachment. Patients used the PCCS for 7 days post surgery to assess their symptoms and signs each day after receiving an alert. Data were automatically collected using a cloud computer system, including symptom grades and messages sent to medical staff for further medical assistance. A user satisfaction questionnaire was administered to the patients on the seventh day post surgery.

Results: In total, 185 patients participated in this study. There were 26 (3.03%) reports of symptom grade deterioration (including increased blurred vision, eye swelling, nausea, and floaters or flashes) from 12 (6.5%) patients. We found no difference in the gender of patients who received an early medical consultation. One case of endophthalmitis was reported, wherein an improvement was observed after prompt administration of IVI antibiotics twice. Overall, 87% (n=185) of patients were satisfied or very satisfied with communicating their symptoms instantly through the app; they were willing to use it again and believed that it could improve the quality of care. Seven of the 185 (3.8%) patients had an earlier medical consultation and 1 (0.5%) had endophthalmitis.

Conclusions: The chatbot system, designed for self-reporting postoperative symptoms and providing instant bidirectional feedback on smartphones, could be beneficial for enhancing the quality of care in early medical consultations without gender differences among patients with AMD receiving IVI, and achieved satisfactory responses from patients.

Introduction

Age-related macular degeneration (AMD) is the leading cause of vision loss in patients older than 65 in the many countries [ 1 ]. Repeated intravitreal injection (IVI) of anti–vascular endothelial growth factor (VEGF) antibody is the gold standard and most common surgical treatment for AMD [ 2 ]. Clinical trials using monthly fixed doses have shown the best results, but findings are unavailable from real-world settings and are compromised owing to the use of less common strategies such as pro re nata and treat-and-extend [ 3 , 4 ]. Rare but serious postoperative complications leading to blindness may occur after IVI, such as endophthalmitis and retinal detachment [ 5 , 6 ]. The incidence of endophthalmitis after IVI is approximately 1 in 5000 patients (0.02%). Certain ocular signs and symptoms are associated with post-IVI ocular inflammation, including redness, tenderness, blurred vision, floaters, photopsia, or visual field defects. Early detection of associated symptoms and signs and prompt management could achieve better outcomes [ 7 , 8 ]. However, most patients receiving IVI of anti-VEGF antibody in the outpatient setting and in follow-up visits are ordinarily scheduled for assessment 1 month later to alleviate the follow-up burden resulting from a large number of injections [ 9 ]. There is a gap, which may delay treatment past the optimal window [ 10 ], especially during the COVID-19 pandemic. To support patients during this period, we developed a chatbot self-report system to provide an alternative way to monitor patients’ conditions.

In this study, we assessed the usability and feasibility of a smartphone-based postoperative care chatbot system (PCCS; Bipower Co) for assisting patients who received IVI. The PCCS periodically prompted patients to self-record and self-report post-IVI symptoms and signs and was equipped to send alerts automatically to medical staff for prompt feedback.

This retrospective cohort study was conducted at the Kaohsiung Chang-Gung Memorial Hospital in Taiwan. Patients who received IVI for AMD were recruited consecutively. The excluding criteria were not having a smartphone, not using the LINE app, being illiterate or unable to communicate in Mandarin, or being unwilling to use the PCCS.

The PCCS app was developed using the program code released from the LINE app (LINE Corporation) for both Android and iOS. Patients using the LINE app could use the PCCS app by scanning a QR code, and their data were securely stored in a cloud database. During the study, the app was securely linked with the data server of the cloud computer server in accordance with privacy protection standards mandated by the Taiwan Personal Data Protection Law.

Patients undergoing IVI and having a smartphone with an Android or iOS operating system and the LINE app installed were considered eligible. Patients were asked to participate in this study during their visit to the outpatient presurgery clinic based on their consent. In addition, the researcher helped the participants to use the app on their smartphone and provided verbal and “hands on” instructions on its usage and the course of the study. In brief, after downloading the LINE app from Google Play or App store, the patients were asked to allow the app to store data and access messages and contacts upon opening the app. Thereafter, the patients had to register an account by inputting their name and joining the PCCS group by scanning a QR code. Upon joining the PCCS group, they were required to input the date of IVI and select their ophthalmologist’s name. Patients’ account data were linked with the cloud database.

Once the patients had left the postoperative care room after receiving IVI, they were able start using the app to self-report any postoperative symptoms and signs ( Figure 1 ). In addition, all patients received a notification every day and six questions ( Figure 2 ) about their condition: (1) Eye pain? (2) Eye redness/swelling? (3) Vision blurring? (4) Headache in the side? (5) Nausea or vomiting? (6) Floaters, or flashlights? Each question had four potential answers: (1) Better, (2) Same or none, (3) Not good, and (4) Worse. If the patient chose the “Worse” option in any question, the app would send a notification to the medical facility to decide whether a face-to-face examination in the outpatient department (OPD) or emergency room (ER) was required. A questionnaire regarding satisfaction was automatically presented in the PCCS app on the end of the seventh day post surgery to collect the patients’ feedback, which included the following three questions: (1) Do you feel satisfied with using the PCCS to report ocular symptoms instantly? (2) Do you think that the PCCS is helpful to improve the quality of care? (3) Are you willing to use the PCCS again in the future? Each of these questions had three response options for patients to choose: (1) Satisfied, (2) No comment, and (3) Unsatisfied.

Ethical Considerations

The study was approved by the institutional review boards of Chang Gung Memorial Hospital (202201604B0). Written informed consent was obtained from all participants.

A total of 185 patients (87 women and 98 men) used the PCCS app after IVI. Among them, 53% (n=185) answered the app questions by themselves, 34% had their children’s respond, and 13% had others respond.

A total of 858 responses were obtained from the patients. There were 26 (3.0%) reports among 12 (6.5%) patients who chose the “Worse” option once during the 7-day follow-up. Deterioration was reported 2.16 (range 1-7; 26 divided by 12) times on average per patient, and there was no significant difference in the gender of these patients (chi-square P =.83). Six patients reported deterioration of blurred vision, 3 with increased floaters or photopsia, 2 with eye swelling or redness, and 1 with nausea or vomiting. Messages were sent to the medical staff, and the patients were contacted by phone for further OPD or ER visits. Seven of 12 patients returned to the OPD earlier after having been suggested to do so during the phone consultation. One patient with a history of diabetes mellitus reported deterioration of blurred vision and was diagnosed with endophthalmitis. Ocular inflammation and infection subsided after prompt IVI of antibiotics. Seven of the 185 (3.8%) patients had an earlier medical consultation and 1 (0.5%) had endophthalmitis.

The post–PCCS app use survey over 7 days shows that 87% (n=185) of the patients were satisfied with the use of the PCCS app to communicate their symptoms instantly; most felt that it was helpful to improve the quality of post-IVI care and were willing to use it again in the future.

In this study, the chatbot system detected 4% of patients needing earlier medical consultation without gender differences, of whom 0.5% required urgent management within 7 days post IVI. Patients reported being satisfied with the app’s enhancement of the quality of care.

Large retrospective studies have reported that around 0.6%-1.9% of patients seek urgent or unscheduled visits within 1-2 weeks after IVI, which is lower than what our study reports [ 8 , 11 ]. Gender differences in seeking medical consultation have been reported [ 12 ]. The chatbot system may enhance detection and eliminate gender differences.

Acute postoperative endophthalmitis frequently occurs within 1 week post surgery. Early detection and prompt management could possibly achieve better outcomes [ 7 , 13 ]. In contrast, a treatment delay may lead to irreversible vision loss. In this study, due to early report of deteriorated blurred vision from a patient, prompt medical intervention was provided, and the patient’s vision was salvaged after emergency treatment.

A new anti-VEGF antibody, brolucizumab, has reportedly shown adverse reactions in intraocular inflammation (IOI), such as retinal vasculitis and retinal vascular occlusion [ 14 ]. IOI occurrence should be alerted in patients treated with brolucizumab 6 mg (HAWK study: 5.3%; HARRIER study: 2.7%). Therefore, more frequent follow-up and prompt treatment are suggested for those receiving IVI of brolucizumab. The PCCS app might be a suitable choice for follow-up to promptly report the possibility of IOI after IVI of brolucizumab.

This chatbot system could thus be considered a supplementary telehealth option for postoperative ophthalmologist-delivered care to help monitor patients remotely. With the assistance of artificial intelligence and instant feedback without delay, physicians can better individualize the care of patients and treatment suggestions, and minimize clinic visits. It can also improve patient care and reduce costs after IVI, especially for those who live in rural areas. It can also be a potential solution in situations where fewer visits are possible, such as during the COVID-19 pandemic to minimize potential exposure to SARS-CoV-2. Furthermore, this PCCS app is potentially applicable in any kind of surgery to offer holistic care to patients. It could also contribute to improvement of the doctor-patient relationship.

This study has some limitations including a small sample, its retrospective nature, and the lack of a control group. Some older people or those with poor vision would be unable to use a smartphone and may need a family member’s assistance.

This smartphone-based chatbot system designed for self-reporting postoperative symptoms and providing instant bidirectional feedback could be beneficial for patients with AMD, enhancing early medical consultation without gender differences. It achieved satisfactory responses from the patients. It can potentially improve the quality of patient care after IVI and may be applicable to other surgical procedures.
